Note on bleeding: It is very common with Zoely to have irregular bleeding or no bleeding at all (amenorrhea) during the placebo days. As with all combined pills, there is a small risk of blood clots.

Zoely is a next-generation combined oral contraceptive pill that differs from traditional pills by using "body-identical" hormones. It contains estradiol, an estrogen that is structurally identical to the hormone naturally produced by your ovaries, combined with nomegestrol acetate.

Because of this unique composition, Zoely is often prescribed to women who experience side effects with synthetic estrogens. It follows a continuous 28-day cycle (24 active white tablets and 4 inactive yellow tablets). This regimen helps maintain stable hormone levels and typically results in shorter, lighter withdrawal bleeds, or sometimes no bleeding at all.

Ingredients

Active tablets (White): Nomegestrol acetate (2.5 mg) and Estradiol hemihydrate (1.5 mg). Inactive tablets (Yellow): Placebo (contain no active substances). Excipients: Lactose monohydrate, microcrystalline cellulose, crospovidone, talc, magnesium stearate, colloidal anhydrous silica.

Side effects

Nausea, headaches, breast tenderness, mood changes, or light breakthrough bleeding between periods may occur.
Less commonly: weight gain, reduced libido, irritability, or digestive disturbances.

Serious but rare side effects include deep vein thrombosis or pulmonary embolism. Seek immediate medical attention if you experience leg pain, sudden shortness of breath, or chest pain.

When should I start taking the pill?

You should preferably start taking Zoely on the first day of your menstrual bleeding. You are then protected against pregnancy immediately. If you start on days 2–5 of your cycle, you must use extra protection (like a condom) for the first 7 days.

Can I skip or delay my period with this pill?

Yes. To delay your period, finish the 24 white active tablets in your current strip and discard the 4 yellow placebo tablets. Start the white tablets from a new strip the very next day. This allows you to skip the withdrawal bleed. You may experience some spotting during the second strip.

Does this pill protect against sexually transmitted infections (STIs)?

Zoely prevents pregnancy but does not protect against HIV or other sexually transmitted infections. You should always use condoms to protect against STIs.

Which medications can reduce the effectiveness of this pill?

Certain medications can interfere with Zoely, including treatments for epilepsy (e.g., carbamazepine, phenytoin), tuberculosis, HIV, and herbal remedies containing St. John’s Wort. Always inform your prescriber of any other medicines you are taking.

Can I take this pill while breastfeeding?

No. Zoely is generally not recommended for use during breastfeeding. Although it contains natural estradiol, it can still affect milk production. Consult your doctor for a progesterone-only alternative.
